Infant, and Children
(WIC) Certification
The
Women, Infants, and Children Nutrition Program provides nutrition
education and supplemental foods for pregnant and breastfeeding women,
infants, and children under five years of age. What
does the client need to bring to the appointment?

ALL
clients applying for WIC must be present at the time of certification
-
Bring
proof of ALL income (before taxes and deductions) received by ALL
persons living in the household
-
Bring
one proof of Identification for each family member applying for WIC
such as:

Birth
certificate

Valid
driver's license

Current
Work or School ID

Current
Military ID
-
Health/Social
Services ID Card
-
Discharge
Papers or Crib for babies less then 2 months

Utility
bill - electric or phone
-
Housing
or rental agreement
-
If
living with another person, bring proof listed above in that
person's name and a letter from that person stating you live with
them.

Security Card
-
Immunization
records for children
-
Florida
WIC Medical Referral Form completed by the doctor or health center

Bring
your WIC card only if you have one
