Community-Based Participatory Research & Practice-Based Research Networks

Public Health Practice-Based Research Networks: Public Health Practice-Based Research Networks is a national program of the Robert Wood Johnson Foundation (RWJF) that supports the development of research networks for studying the reach, effectiveness, efficiency and equity of public health practice. A practice-based research network (PBRN) brings multiple public health agencies together to design and implement studies in real-world practice settings. Findings will advance RWJF’s efforts to enhance the evidence base for public health policy and practice.  For more information on the PBRN click on the PBRN Fact Sheet link.

PBRN Regional Statistics Report: 5 FL counties - partnership w/Center for Health Statistics - 1st project for NEFL consortium.

RWJ Quick Strike H1N1 Study: Describe "best practices" in the FL County Health Department H1N1 response - statewide survey, contextual focus "look backs" in Northeast Florida.

 

Evaluation

Regional Medical Public Health Education Center (RMPHEC): Grant supported to educate Medical Students in Public Health.

Evaluation of Men's Health Summit (2010):

Fresh ministries/Cease Fire Funding Pending: A formative evaluation will be conducted by DCHD. Program staff and volunteers will be recruited and trained. The CeaseFire model will be adapted to the local context and implemented. CeaseFire Chicago will provide consultation on these activities.

 

Health Information Exchange

Northeast Florida Health Informatics Consortium (NEFHIC): Establish a community HIE infrastructure in Duval County and its surrounding area.

Jacksonville Health Information Network: The JHIN is a health information exchange (HIE) initially focused on improved coordinated care for Northeast Florida’s under- and uninsured population.

Rural and North Florida Regional Extension Center (REC):

The RECs will support and serve health care providers to help them quickly become adept and meaningful users of electronic health records (EHRs). RECs are designed to make sure that primary care clinicians get the help they need to use EHRs.  

RECs will:
  • Provide training and support services to assist doctors and other providers in adopting EHRs
  • Offer information and guidance to help with EHR implementation
  • Give technical assistance as needed

Beacon Communities Cooperative Agreement Funding Pending: This agreement program provides funding to communities to build and strengthen their health information technology (health IT) infrastructure and exchange capabilities.  These communities will demonstrate the vision of a future where hospitals, clinicians, and patients are meaningful users of health IT, and together the community achieves measurable improvements in health care quality, safety, efficiency and population health.

Black Infant Health Initiative: The purpose of this project is to increase community awareness of the high infant mortality rate in the Black community and engage the community in addressing the issue.

Community Asthma Partnership @ Wolfsons: A surveillance and formative evaluation project used to assess a pediatric community based asthma management program

RWJ - Quality Improvement Project (Immunization): Evaluating Quality improvement strategies to improve immunization rates of children 2 years old or younger.

Customer Experience Survey (2010): Develop a customer satisfaction program that will enhance the customer experience at DCHD.

DD Claims:  AHCA funded multiple projects (subcontracted through UF CPHHP) renewed annually to analyze Medicaid claims data in Florida to answer research questions that are established annually.

Dental “Healthy Smile” Focus Groups:  Private foundation funded project to assess barriers to utilization and awareness of importance of dental health services for children.

Diabetes Translation Research Study: Analysis of AHCA morbidity and mortality Data

Dyson Evaluation: Formative and summative evaluation of a 2.5 million dollar project to train pediatric residents in community pediatrics.

Family Nurse Care Coordination Partnership: An NIH sponsored R21 research (planning) project to study the role of Nurse Care Coordinators impacting health and quality of life issues of children with special health care needs.

HIV Prevention-Selective Exposures: A sample of at-risk participants will be recruited through collaboration between UIUC and the Duval County Health Department for a randomized, double-blind trial. The study will investigate if a meta-intervention video designed for empowering participants as agents of their own change can increase the number of attended sessions relative to a control condition without such a video. This trial will also determine if a meta-intervention video addressing various emotional/social and instrumental benefits of an HIV-prevention-counseling intervention can also increase the number of attended sessions. These two factors will be crossed, and their effects on retention will be estimated for different genders and ethnicities.

Jaguars Evaluation: A community-based utilization focused evaluation of local NFL Foundation media and agency programs to reduce teen pregnancies and STDs.

JailLinc Evaluation:  Evaluation study of a HIV prevention and treatment programs for people housed in the county jail (CDC funded).

Magnolia Evaluation: An Evaluation Study to assess the Impact of Pre and Interconceptional Care Case Management on Birth Outcomes of High-Risk Women.

Medicaid Pilot Eval: Community based initiative to evaluate the impact of Medicaid reform on local community (internal funded).

Medical Home Evaluation: Evaluation of Medical Home Intervention for Children with Special Needs: The major goal of this project is to evaluate a medical home, care coordination approach to providing services to children with special needs (DOH-CMS funded).

Medicaid Reform: Policy and evaluation monitoring. Updating affected DCHD constituents.

Mental Health Systems Evaluations

Neighborhood and Mammography Project: The intent is to achieve a better understanding what these women might themselves identify as factors at multiple levels of influence (e.g. individual, neighborhood- rural vs urban) that contribute to the lower rates of mammography among African-American women which in turn contributes to cancer mortality.

Obesity Policy Evaluation: Follow up evaluation study of a community-based initiative to recommend evidence based policies to reduce the obesity epidemic in the local community (internal funded).

Obesity: Development of obesity policy report. Participation in various community coalitions related to Obesity policy, research and grant work. Working with Healthy Jacksonville on the Embrace a Healthy Florida grant, data surveillance and evaluation committee.

Palliative Care Evaluation:  A local foundation supported evaluation project to assess the impact of a palliative care program on children with terminally debilitating chronic conditions, as well as their families and the institutions including utilization and cost of hospital services (Local foundation funded).

Physical Education Evaluation: Evaluation of Physical Education Program:  The major goal of this project is to evaluate a 1.4 million dollar effort to increase physical activity in public schools (US Dept of ED funded).

Prostrate Cancer Awareness Program:  Evaluation project for local prostate cancer education and awareness program (internal funded).

Public Health Informatics Network: This grant will support development of a manual/guide for local health department (LHD) leadership in coalition development to build social and political capital among local and regional public health, clinical care and emergency response organizations necessary for the development of HIE. The manual will contain guidelines for eHIE development using coalition strategies and assessment tools to aid in implementation of the guidelines. The manual will also include foundations in public health principles and practices for LHD involvement in HIE.

Safe Sleep evaluation: Evaluation project for local education initiative.

Social Capital-Immunization: Study of relationship of social capital to immunization rates among low income high risk populations.

Synagis: Study of the factors influencing access to Synagis vaccine by high risk low income premature children.

Tobacco Evaluation: The goals of the Tobacco Prevention Program are to assist the Smoke-Free Jacksonville Coalition in changing the community environment to discourage tobacco use; train and empower youth to change community attitudes and behavior about tobacco and de-normal.

Tomorrow’s Rainbow: Evaluation project for breast cancer mammography screening education and outreach program (internal funded).

Transdisciplinary CBPR Approach to Translational Research to Reduce Diabetes Rates: The transdiciplinary team approach includes a major emphasis on community-based practitioners having a major role in the research leadership, collaborating with accomplished researchers and community residents.

TRAQ-Transition Readiness Assessment Questionaire Mobilizing Assessment Perinatal Periods of Risk (PPOR): TRAQ was based on a review of the existing literature and on a theoretical framework of skills needed to navigate health care transition successfully.

Unnatural Causes/Health Equity/Health Impact Assessment: Exploring opportunities to increase awareness of health equity issues internally and externally. Educating and advocating for integration of health impact assessments into regional/city wide planning and decision making.
