DCHD Transition FAQ What specific services will be affected by DCHD changes? Women’s health services (family planning & prenatal care) will no longer be provided at our Marietta Health Center and Center for Women and Children locations. Adult primary care services will no longer be provided at our West Jax location. Women’s health services will be expanded to the Agape Community Health center. Who will be specifically affected? Adults who receive medical care services at our West Jacksonville health center will encounter changes to our system of care. If you are unsure of your status, please contact us at 904-253-1000 prior to your next appointment. When will these changes take place? We are beginning the process of implementing these changed immediately with the expectation that we will have completed the transition by September 1, 2010. How are affected adult patients being notified? Potentially affected patients will receive a letter. Patients will also be notified as they schedule appointments for services or as they attend scheduled visits with providers DCHD has also set up a dedicated phone number and website for questions. What can adult patients do to assist with their transition? If you are affected by these changes, please make plans to transition your medical care to an alternative DCHD health center or community resource. Those community resources can be found at the front desks of all DCHD clinics and on our website at www.dchd.net. What does this mean to other services offered at these and other health center locations? DCHD will continue to provide services vital to the health and safety of the community, including Family Planning, Pediatric Care, STD Testing and Treatment, Immunizations, Prenatal Care, and limited adult primary care. Will affected services be restored at these locations, if so when? These changes will remain in place for the foreseeable future. What help is being offered to affected patients? DCHD will assist all patients with identifying other primary care options in the community, including other DCHD clinics. We will also transfer the medical records of patients to their new medical home upon request. During the transition, DCHD will continue to provide for client’s urgent care, prescription refill and referral needs. Where else can clients who are being transitioned go for services? We are making available to list of community health resources and eligibility requirements to assist with the referral process. Other changes may be announced as we continue to evaluate our system for continued improvement. Why are these changes necessary? Due to significant reductions in state and local funding, it became clear that services had to be consolidated so as to ensure that the vital care that the DCHD provides to the community could continue. How is DCHD staff assisting with these changes? Staff and management are prepared to assist clients with questions and referrals regarding services provided at our health centers. Our eligibility counselors stand ready to help the community identifying other health care resources either within the DCHD system, or out in the community. They can be reached at 904-253-1360.
