Changes
to
DCHD Health Care Services
Please note the following actions as DCHD and Agape Community Health Network merge locations of some health care services:

Beginning August 1st we will no longer offer adult primary care services at West Jax Health Center. If you are a patient utilizing adult primary care service at this location, we are asking that you transition your health care services to an alternative DCHD or Agape health center or to other community resources.

Beginning September 1st we will no longer offer Women’s Health services at Marietta Health Center or The Center for Women and Children. If you are a patient utilizing women’s health services at these locations, we are asking that you transition your health care services to an alternative DCHD or Agape health center or to other community resources.

If you are affected by either of these changes, it is very important you make plans to transition your medical care before your next appointment. If you select to continue receiving your care within our network of health centers you can make an appointment by calling us at 253-1130.
To assist you in this transition, and make certain you are able to receive consistent and stable care, we will transfer your medical records to the new medical provider upon request.
