The Duval County Health Department (DCHD) invites you to take advantage of our Medication Assistance Program.

Most pharmaceutical companies offer medication assistance to low-income individuals. We can help you take advantage of these programs and get the prescription medication for you and your family at a lower cost.

You may qualify if:

  • Your insurance does not pay for your medication.

  • The high cost of medications causes you a financial hardship.

Enroll today:

  • Get your prescription(s) from your Doctor or Health Care Provider

  • Call the DCHD at (904) 253-1059 before getting your prescriptions filled to see if you qualify

  • Get an enrollment application here, from the Duval County Health Department, or your Doctor or Health Care Provider

  • Complete the application, mail or deliver to:

    Central Pharmacy

    Jacksonville, FL 32206

    or fax to 253-1955

  • Follow up with DCHD by calling 253-1059.

  • Pay a small application fee

  • We will take care of the rest
